Achieve participants receive the following kinds of
personal and social support:
Participants whose program goals include
increasing their independence, will regularly go into the community
with staff and engage in activities with the general population.
Becoming accustomed to appropriate interactions with people they
don’t know and learning about the community are benefits of
activities like visiting a restaurant and making a purchase at a
store in the mall.
Achieve staff formally collects regular
feedback from participants, their families and legal guardians, or
caretakers, and presents it to the Achieve Advisory Board. The board
uses this information to guide management in determining the
services it will provide participants and to makes changes in
service delivery.
Although they’ll meet as needed, typically,
a participant’s interdisciplinary team meets twice a year. The team
consists of the participant, Achieve staff, residential staff,
family members, guardians and other interested persons. The team
evaluates the participant’s progress, discusses his or her goals and
their satisfaction with current services and assignments. The team
seeks information on expanding interests and the need for therapy or
additional training.
